How to fill in the online health form and get the Peruvian health affidavit

Until October 12, 2022 everyone - visitor, resident and Peruvian; adult and child; vaccinated or unvaccinated - who came to Peru by air was required to present a Health Affidavit when entering the country. This changed with the Ministerial Resolution 811-2022-MINSA.

So, since October 12, 2022, the Health Declaration is no longer required to enter Peru! The website to fill in the form has been deactivated as of October 20, 2022.

As Peru is still in a state of (national health) emergency and prevention and control measures to stop the spread of Covid remain in place at least until February 24, 2023, things can be tightened again in no time and the health declaration might be necessary in the future again. So, for now, we leave our guide to fill in the form below.

 

Completing the online form takes less than 10 minutes, is super simple, free of charge and at the end you immediately get the required health affidavit which you can either download or print. The form can be filled in up to 72h before entering Peru (within 72 hours prior to your arrival in Peru). You might be asked to show the health affidavit when checking in to your flight to Peru and/or at the airport in Peru and/or when traveling.

Please only use the official website for obtaining the health affidavit and don’t fall for a number of service providers offering to get the sworn statement for you while charging you anything from US$ 20 up to US$ 60 for something you can easily do yourself in a few minutes for free.

How to complete the online form and get the Peruvian health affidavit

If applicable, before you start switch off your VPN and any translation program that might run on purpose or automatically!!!

Step 1

Visit the official health affidavit website.

There have been reports that the page doesn’t always load correctly in some browsers, an error message pops up and people can't fill in the required fields on the first page. If you are using a VPN, switch it off, reload the page and it should work smoothly. Otherwise, try to use a different browser; we had no problems with Chrome, Firefox and Safari.

Additionally, we get many questions that people can't find nationalities or states or cities in the drop-down lists on the first and second page. Note that the names are always in Spanish (see Step 2)! If you see them translated, switch off any translation program that may run in the background automatically by your browser or intentionally otherwise error messages can pop up or you won't find the correct names.

Step 2

At the bottom of the first page you can choose between English and Spanish, however while the labelling of the spaces you have to fill in changes from Spanish to English and vice versa, the drop-down list under Nationality remains in Spanish; so if you are, for example, a US national you won’t find United States or US on the list, but have to click on EE.UU (Estados Unidos, the Spanish name for the US); or if you are German, click on Alemania, etc. Those from the UK will look in vain for United Kingdom but as well for Reino Unido; select instead Gran Bretaña  (Great Britain) or, if you are from England, you could choose Inglaterra, or, if you are from Scottland, Escocia.

Then choose the document with which you are travelling: DNI, CE or PAS (passport) and enter the number. Complete the first page by adding your birthdate and then click on “Search”.

Step 3

The second page of the Peruvian online form to get the health affidavit is divided into two parts:

  • Part I: Personal Information
  • Part II: Health information

So, first fill in your personal information exactly as in the document you are traveling with, your arrival date and flight number. Under “Country of origin” (the country from which the flight departs to Peru) the names are again in Spanish (see step 2).

Then choose your destination in Peru. You are asked for the department (Peruvian state), the province and the district; so, for example, if you stay in the district of Miraflores in Lima, choose Lima (department), Lima (province) and Miraflores (district) or if you travel to Machu Picchu choose Cusco (department), Urubamba (province), Machu Picchu (district). If you are traveling around, either choose your first destination or the place where you stay the longest.

Now add a contact phone number and a cell phone number. Be aware that fields for the (cell) phone number are Peruvian-style, so, if necessary, you might have to "adjust" your phone number to the Peruvian structure.

  • the country code must have 2 or 3 digits
  • the city / area code must have 2 or 3 digits
  • the phone number must have at least 6 digits, the cell phone number at least 8 digits

Example: let’s assume you have a US cell phone number which usually is structured as follows: +1 (country code) 123 (area code) 1234567 (number). This cell phone number won't be accepted as there are two problems with it: the system doesn’t allow the “+” and the number only has 7 digits. So, just enter in the country code field 01, in the city code field 12 (the first two digits of your area code) and in the number field 31234567 (last digit of your area code and then the 7 digits of your number). Problem solved.

Then enter a valid e-mail address you have access to even during your travel.

The following fields seem to be aimed at Peruvian travellers (and in an earlier version of this form were in a section which only Peruvians had to fill out) and don't really fit international tourists.

You are asked for your "real address", but you can only choose a Peruvian department (state), province and district. So, just as above, where you had to choose your destination in Peru, use the address of your hotel or the address of friends / family you are staying with. If you are traveling around, choose the first hotel you are staying at or the place where you stay the longest.

And the next field seems even stranger; you are asked to enter your "real address after isolation". This just makes little sense for foreign travellers, so just enter the street address of your accommodation.

The next fields aren't better as you should add the persons who are with you at your "real address". We recommend to either leave these fields blank (each person traveling should have his or her own health declaration anyway) or enter the information of whoever is traveling with you. After entering the data of a person, you have to click "Add", otherwise the system won't accept the entry.

Step 4

In Part II Health Information, as of May 2022, you just have to answer three Covid-related questions and then tick the box before the declaration of oath; the geolocation authorization for your cell phone, which never had a legal basis in Peru, is gone.

Click registrar, and you immediately get the Health Affidavit. At the top of the page, you can and, as once you leave this page you can't return, should download ("descargar") and/or print ("imprimir") the form. Sometimes, if you have selected "imprimir" the window that opens remains blank. Just shut the print window with the "x" at the top right, choose the "descargar" button and print from there.

    My travel company, Exoticca, has told me I need a health declaration from Peru.  When I load the link https://e-notificacion.migraciones.gob.pe/dj-salud/  my Chrome browser says "This site can’t be reachedCheck if there is a typo in djsaludviajero.minsa.gob.pe.
    DNS_PROBE_FINISHED_NXDOMAIN" 

    Is the health affidavit still not required? I cannot find any up to date info thanks !!

      seems your travel company isn't up-to-date.

      Already since October 12, 2022, the Health Declaration is no longer required to enter Peru! The website to fill in the form has been deactivated as of October 20, 2022.

      Additionally, there are no further health related requirements to enter Peru.

      Since October 12, 2022, the Health Declaration is no longer required to enter Peru! The Geolocation Authorization, which only was one question on the Health Declaration form, was already eliminated a few months before October as it's unconstitutional.

      Anyway, no, there are Covid related requirements to enter Peru anymore.

      as mentioned at the top of this article and as I already explained to Gary, since October 12, 2022, the health declaration is no longer required to enter Peru. Therefore, the website to apply for the health affidavit was deactivated.

      To pre-register your arrival in Peru you have to download the app “Pre Registro Migraciones”. In our article “Pre-Registration at Peruvian Immigration before arrival” we explain in detail how it works.

      Please be aware that pre-registering is not obligatory. Many travelers have problems downloading the app or filling in required fields. Additionally, the automated passport control machines that pre-registered travelers were supposed to use are still out of order and everyone has to proceed to an immigration counter anyway.

      So, trying to get the app work and filling in your data might not be worth the effort.

      yes, the link was deactivated.

      As mentioned above, you no longer need the health declaration to enter Peru, and seemingly all airlines have been informed about this change. So, no need for the website anymore.

    I have tried to complete the first page of the declaration and it appears it doesn't like my passport number.  I'm American and have used ee.uu in the nationality field.  I chose PAS and entered my passport number in the Number of Document field. When I hit Search, the field where I put my passport number turns red.  What am I doing wrong?

    I was on another website earlier and found a statement that said as of October 12, 2022 the Peru Health Declaration is no longer needed.  Unfortunately I cannot find this website again.  Is this a true statement ?  If so, I don't need answers to the previous paragraph.

      yes, there have been rumors for a week now that the Health Declaration is no longer required, but I couldn't find any official publication until yesterday evening.

      According to the Ministerial Resolution 811-2022-MINSA published by the Peruvian Ministry of Health, the Health Declaration is no longer required to enter Peru. However, this news didn't reach all airlines flying you into Peru, who checked the form for the past two years and didn't let anyone board without it.

      So, I hope your airline is informed about the changes.

    Hello, I am concerned about answering "yes" to whether I have a cough.  I had COVID-19 in early April 2022, cleared the infection, tested negative multiple times since then, but have had a lingering cough (probably "long covid").  The cough usually occurs when I've been talking a lot.  No other COVID-19 symptoms.  Fully vaxed (2 Moderna) and boosted (2 Moderna), even got the updated bivalent booster a week ago.  Do you think that I'll be denied boarding or entry (just transiting through LIM on the way from U.S. to Sao Paulo, Brazil)?  I had no problem entering Brazil in July (non-stop flight from U.S.), so I'm only concerned about Peru's policies.

    Also, in this circumstance, should I get a COVID-19 test before departing the U.S.? I have an antigen test kit with me. And I will be wearing an N95 mask during the flights and in the airports, if that matters.

      First of all, I combined both of your comments to one.

      It’s completely up to you how you fill in the health declaration form. While you should be aware that it’s a declaration on oath, the questions are intended to figure out if the traveler might have a possible acute (!) / contagious infection with Covid. According to your description of your state of health, that clearly doesn’t seem to be the case.

      Therefore, if you decide to answer no, that wouldn’t be wrong. If you, however, answer yes, you still get the health declaration. But the airline who usually checks the health declaration during the check-in process to flights to Peru might question you, might ask for proof that you don’t have Covid, might deny you to board; or they don’t.

      If you stay in transit in Peru, no-one will even check your health declaration; if you have to go through immigrations and customs at the airport and then check in to your flight to Brazil, then your health declaration might be thoroughly checked before the immigration counters (often times not), and you might be questioned; but you have 4 Covid shots and most probably in Peru no-one will bother. However, the Peruvian authorities have the right to mandate that you to take a Corona test in case they think you could have Covid, no matter if you are vaccinated and no matter if you have a current negative test result.

      As you already have 4 shots of a vaccine against Covid, according to the current regulations you do not need a negative PCR test neither to board nor to enter Peru. If you feel better having one, go for it. Antigen tests aren't accepted.

      Additionally, on international flights to and from Peru a N95 mask is no longer required. If you feel safer wearing one, do it.

      For the past months, the health declaration isn’t thoroughly checked anymore when entering Peru. But, usually during check-in to your flight to Peru, the airline checks the form and there someone might (or not) stumble upon the wrong date.

      So, to avoid any inconveniences or delays, yes, I recommend filling in the health declaration again with the correct arrival date.

    Hi, when I put in the German passport number - which is a 9 item letter-number combination, I get the failure message Sololetras y numeros. well, iI only put in letter and numbers. I tried out 6 numbers, and then it works, but German passports have 9 numbers and letters. How do you overcome that problem? I tried different browers and different devices, but nothing works.

      that's really strange. I'm German and have a German passport with a 9 digit letter number combination, so do have my kids.

      I just went to the health declaration website and tried out our three German passport numbers. All three were accepted. So, I don't know what the problem is.

      If you can't make it work and trust me enough, send me your personal data and everything else using our contact form. I'm more than happy to try finding out why it doesn't work, hopefully will be able to fill in the form for you and can then send the declaration to you.

    Hi.Due to fly LHR - MIA - LIM.To be absolutely certain that there will be no issues with our vaccination certificate I plan on taking a PCR- RT test in London.Am I correct in thinking that  the 48 hour limit for the test result is calculated based on  the  local time in MIA rather than the result time in London?Is the time limit calculated to the time I present the test at boarding in MIA or the scheduled departure time?

      As the flight bringing you to Peru starts from Miami and not Heathrow, the 48h are calculated when you check-in to your flight in Miami.

      Article 4.3 of the current Covid regulations says that those not vaccinated according to the rules can present a negative real-time COVID-19 molecular (RT-PCR) test result that is not older than 48 hours after being issued and before boarding the plane (“De no contar con la citada vacunación, pueden presentar una prueba molecular negativa con fecha de resultado no mayor a 48 horas antes de abordar en su punto de origen.”)

      When I remember correctly, the time difference between London and Miami is 5 hours. So, let’s assume you have a PCR test result from the UK from November 1 at 02.00 pm. According to the Peruvian regulations, the latest you can get on board with this test result is November 3 at 02.00 pm UK time, which would be November 3, 09.00 am Miami time. I can’t tell you if the airline takes the check-in time, the boarding time, or the departure time. Usually, they use the time when the vaccine certificates / PCR test results are checked which usually is when you check-in to your flight. The regulations says “antes de abordar”, so before boarding. If you want to make sure, probably best ask the airline.
